TB Services for Aboriginal Communities (TBSAC) was transferred to First Nations Health Authority (FNHA) on April 1, 2016. The new FNHA TB Services webpage with links to educational resources and TBSAC specific forms can be found here. Please update your bookmarks.

Medication sheets remain on the BCCDC website.

BCCDC clinical forms are included below. As of April 1, 2016 health professionals working with First Nations communities are asked to fax all documentation to FNHA TB Services, at (604) 689-3302.

To contact FNHA TB Services, please email or call 1-844-364-2232.

The forms are in "fillable format", so information can be recorded directly onto the forms and saved from the website.


Posters are now available through FNHA TB Services. The links below will direct you to FNHA’s page where you may access the most updated materials.


Some videos are available through FNHA TB Services, while others are BCCDC TB Services resources. FNHA resources are identified and link to the FNHA website where you can access their materials.
